1. A copy of your USD and CAD bank accounts (void cheque, bank statements, or pre-authorized debit form). Both accounts must be in Canada and must show your name, account number, and transit number.
2. Government issued valid ID (Driver’s License, Passport, Nexus Card, Permanent Resident Card).
3. Current bill showing your name and current address.
4. Please fill out and sign the Pre-Authorized Debit Agreement form that has been emailed to you.
